NOTICE TO VACATE 

This letter serves as official notice that your housing with the Independent Living Program will be ending. 

1. Effective Date of Vacate 

You are required to vacate the program housing no later than: 

All personal belongings must be removed, and the unit must be left in clean condition by this date and time.

3. Program-Based Housing Status 

This housing is provided as part of participation in the Independent Living Program and does not establish a tenant–landlord relationship. Continued housing is contingent upon compliance with program rules and requirements. 

4. Move-Out Expectations 

Before vacating, you must: 

● Remove all personal belongings 

● Return all keys, access cards, or codes 

● Leave the unit free of trash and damage 

● Participate in a move-out inspection if requested 

Failure to comply may result in charges for damages or disposal of abandoned property in accordance with program policy. 

5. Personal Property 

Any personal property left behind after the vacate date may be considered abandoned and handled according to program policy and applicable law. 

6. Acknowledgment of Receipt 

By signing below, you acknowledge receipt of this Notice to Vacate. Signature does not indicate agreement, only confirmation of receipt.
