1. Program Purpose
The Independent Living Program is designed to provide safe, structured housing while supporting residents as they build life skills, personal responsibility, and long-term stability. Housing is a privilege contingent upon compliance with program rules and this Agreement.
2. Term of Residency
Residency begins on [Move-In Date] and continues on a month-to-month basis unless terminated in accordance with this Agreement or program policy.
3. Curfew Policy
● Residents are required to be inside the residence by 11:00 PM each night.
● Late arrivals are not permitted unless prior approval is granted by program staff.
● Failure to comply with the curfew may result in:
○ Verbal or written warnings
○ Loss of privileges
○ Increased supervision
○ Possible termination of housing for repeated violations
● Curfew exceptions may be granted for:
○ Employment schedules
○ Medical emergencies
○ Pre-approved educational or program activities
All exceptions must be documented and approved in advance whenever possible.
4. Resident Responsibilities
Residents agree to:
● Treat staff, housemates, and property with respect
● Maintain cleanliness of personal and shared spaces
● Follow all house rules, program policies, and staff instructions
● Attend required meetings
● Report maintenance issues promptly
● Refrain from disruptive, aggressive, or threatening behavior
5. Prohibited Behavior
The following are strictly prohibited:
● Illegal drugs or substances on the property
● Violence, threats, or harassment
● Theft or destruction of property
● Unauthorized guests or overnight visitors
● Possession of weapons
● Repeated curfew violations
Violation of these rules may result in immediate disciplinary action or discharge.
6. Guests
● Guests are only permitted during approved hours.
● All guests must be pre-approved by staff.
● Residents are responsible for their guests’ behavior.
● Overnight guests are not permitted unless explicitly approved in writing.
7. Safety and Security
Residents agree to:
● Follow safety procedures
● Lock doors and windows when entering or exiting
● Not provide keys or access codes to unauthorized individuals
● Report safety concerns immediately
8. Substance-Free Environment
The residence is a substance-free environment. Use, possession, or distribution of illegal substances or alcohol is grounds for disciplinary action.
9. Termination of Agreement
This Agreement may be terminated:
● By the Program for rule violations or safety concerns
● By the Resident with written notice
● Immediately in cases of serious misconduct
Upon termination, residents must vacate the premises as directed.
10. Acknowledgment
By signing below, the Resident acknowledges that they have read, understand, and agree to comply with this Housing Agreement, including the 11:00 PM curfew.
